The driver of a freight train hauling 25 wagons and a brake van, G.R.K. Speed, was fatally injured when his engine overturned after colliding with the rear of another freight train, just beginning to move off after having been stopped at the Betley Road Home signal. The fireman and the guard of the leading train were injured. The report concludes that driver Speed had passed the Wrinehill Distant signal at Caution without reducing speed and the Home signal at Danger. He had a good record and enjoyed good health, so his attention must have been momentarily distracted. The report recommended the prevention of such accidents by the introduction of the Automatic Train Control system, then under trial.

Tate does not specify whether this is a quotation from the original register. There are several variations from the version in Thicknesse, Ralph, ed., 1916, p.1, where it reads:

To my knowledge there is a copy at the Staffordshire Record Office (a photostat), at Lichfield and at the Birmingham and Midland Institute, Margaret Street Birmingham. The copy belonging to Newcastle-under-Lyme Library is said to have been stolen.
